UK names Canada's Carney to head Bank of England

LONDON (Reuters) - Britain named Canadian central bank chief Mark Carney on Monday as the next Bank of England Governor, springing a major surprise by appointing a foreigner to the job.

Carney, a former Goldman Sachs investment banker who successfully helped to steer the Canadian economy through the global crisis, will succeed Mervyn King next year, Chancellor George Osborne told parliament.

"He is quite simply the best, most experienced and most qualified person in the world to be the next Governor of the Bank of England," Osborne said.

BoE deputy governor Paul Tucker, who had spent his entire career at the central bank, had been the hot favourite to succeed King.

Osborne said Carney brought the skills to revamp British financial regulation at a time when the BoE will take on a new role in change of British bank supervision. Carney had previously strongly played down suggestions he would move to the BoE.

"Mr Carney is unique amongst the potential candidates in combining long experience of central banking, huge international credibility in economics, deep expertise in financial regulation and a first hand experience of private sector financial institutions," Osborne added.

Carney is not a British national, although Osborne said he would apply for UK citizenship, and he studied at Oxford University.

Carney will serve at the Bank of Canada until May, and then starts at the Bank of England in July. He will serve a five-year term, rather than the eight-year term that had been expected for the next BoE governor.

"Surprise, huge surprise," said Peter Dixon, an economist at Commerzbank. "That was the one guy I didn't have in the running, and it's a slap in the face for the likes of (Paul) Tucker and all those other guys who were hoping to step up to the plate."

Mystery of Florida Keys shipwreck solved

For more than 100 years, the sunken ship off Key Largo, Fla., was merely known as "Mike's Wreck," named after the employee of a local dive shop. But a team of archaeologists have finally identified the ship that sunk here in 1911 as the Hannah M. Bell, built in England in 1893.

The vessel once hauled sugar, cotton and other cargo between Europe, the United States and South America, according to a statement from the Florida Keys National Marine Sanctuary, where the Hannah M. Bell rests.

The 315-foot (96 meter) steamship ran aground on a shallow reef known today as Elbow Reef, located about 6 miles (10 kilometers) off Key Largo, on April 4, 1911. The vessel was loaded with coal bound for Vera Cruz, Mexico. For days, salvagers tried to rescue the Hannah M. Bell, but gave up after its holds filled with water, according to the statement. By May, waves had torn the ship apart and left its remnants in shallow water, making it accessible and visible today only to snorkelers and scuba divers. ?

A team of maritime archaeologists took an expedition to the wreck in September, after which they positively identified the ship.

"Similar to the way detectives use forensic information to solve a crime, we compared the dimensions and construction characteristics of the shipwreck known locally as 'Mike's Wreck' with historic shipping records in order to solve this mystery," said Matthew Lawrence, a maritime archeologist, in a statement from the sanctuary. ?"Measurements of the shipwreck and the records for Hannah M. Bell were virtually identical, as were the reported sinking location and the actual location of the wreck."

The steamship was built by Ropner and Son in England and named for the woman who christened it, according to the release.

The Florida Key's shallow coral reefs have claimed many ships over the years, and helped spawn a once-thriving salvaging industry, in which people would reclaim and repair marooned ships and their cargo, both legally and otherwise.

The Hannah M. Bell rests close to two other shipwrecks: the USS Arkansas and City of Washington. The Hannah M. Bell contributed to the loss of another vessel in 1920, when the U.S. Shipping Board steamer Quoque wrecked directly on top of its sunken remains, according to the release.

This led to confusion about the identity of the original wreck, and its name was misidentified in local news reports, said Matthew Lawrence, a maritime archaeologist with the National Oceanographic and Atmospheric Administration. "Considering that there are a number of steamship wrecks on Elbow Reef, exact wreck identities probably slipped from the public's consciousness," he wrote in an email.

Utility worker pierced pipe before Mass. gas blast

SPRINGFIELD, Mass. (AP) ? A natural gas explosion that injured 18 people and damaged 42 buildings in Springfield's entertainment district was blamed Sunday on a utility worker who accidentally punctured a high-pressure pipeline while looking for a leak.

State Fire Marshal Stephen Coan said the Friday night blast in one of New England's largest cities was caused by "human error." He didn't name the Columbia Gas Co. worker who pierced the pipe while responding to reports of a gas leak.

The worker damaged the underground pipe while using a metal probe to locate the source of the leak, Coan said. A flood of gas then built up in a building that housed a strip club, and some kind of spark touched off the blast, officials said.

Coan said the employee was following older markings on a sidewalk that indicated the location of the gas line. He appeared to be an appropriate distance from the line, but the markings were incorrect and the worker accidentally punctured the pipe.

A message left for a Columbia Gas spokeswoman wasn't immediately returned. Columbia Gas, a subsidiary of public company NiSource Inc., announced earlier Sunday that it planned to open a claims center for residents and businesses affected by the explosion at City Hall on Monday.

Preliminary reports showed the blast damaged 42 buildings housing 115 residential units. Three buildings were immediately condemned, and 24 others require additional inspections by structural engineers to determine whether they are safe. The building that housed the Scores Gentleman's Club was completely destroyed.

After the pipe was ruptured, authorities evacuated several buildings. Most of the people injured were part of a group of gas workers, firefighters and police officers who ducked for cover behind a utility truck just before the blast. The truck was demolished.

Some officials said it was a miracle no one was killed. Springfield Fire Commissioner Joseph Conant praised the actions of city firefighters.

"The firefighters did an excellent job evacuating the area which certainly prevented additional civilian injuries and saved many lives," Conant said.

Columbia Gas officials have been cooperating with investigators and have determined that there are no more gas leaks in the neighborhood, Mayor Domenic Sarno said.

Coan said the investigation is being turning over to the state Department of Public Utilities. It's not clear whether investigators will ever be able to determine what caused the spark that ignited the explosion.

Springfield, which is 90 miles west of Boston and has about 150,000 residents, is the largest city in western Massachusetts. It's known as the home of the Basketball Hall of Fame, which is not near the blast site.

The city has been rebuilding from damage caused by a tornado in June 2011.

DHAKA, Bangladesh (AP) ? At least 112 people were killed in a fire that raced through a multi-story garment factory just outside of Bangladesh's capital, an official said Sunday.

The blaze broke out at the seven-story factory operated by Tazreen Fashions late Saturday. By Sunday morning, firefighters had recovered 100 bodies, fire department Operations Director Maj. Mohammad Mahbub told The Associated Press.

He said another 12 people who had suffered injuries after jumping from the building to escape the fire later died at hospitals. The death toll could rise as the search for victims was continuing, he said.

Local media reported that up to 124 people were killed in the fire. The cause of the blaze was not immediately clear, and authorities have ordered an investigation.

Bangladesh has some 4,000 garment factories, many without proper safety measures. The country annually earns about $20 billion from exports of garment products, mainly to the United States and Europe.

Relatives of the factory workers were frantically looking for their loved ones. Sabina Yasmine said she saw the body of her daughter-in-law, who died in the fire, but had no trace of her son, who also worked at the factory.

"Oh, Allah, where's my soul? Where's my son?" wailed Yasmine, who works at another factory in the area. "I want the factory owner to be hanged. For him, many have died, many have gone."

Mahbub said firefighters recovered 69 bodies from the second floor of the factory alone. He said most of the victims had been trapped inside the factory, located just outside of Dhaka, with no emergency exits leading outside the building.

Many workers who had taken shelter on the roof of the factory were rescued, but firefighters were unable to save those who were trapped inside, Mahbub said.

He said the fire broke out on the ground floor, which was used as a warehouse, and spread quickly to the upper floors.

"The factory had three staircases, and all of them were down through the ground floor," Mahbub said. "So the workers could not come out when the fire engulfed the building."

"Had there been at least one emergency exit through outside the factory, the casualties would have been much lower," he said.

Many of the victims were burned beyond recognition. The recovered bodies were kept in rows on the premise of a nearby school.

Army soldiers and paramilitary border guards were deployed to help police keep the situation under control as thousands of onlookers and anxious relatives of the factory workers gathered at the scene, Mahbub said. He would not say how many people were still missing.

Prime Minister Sheikh Hasina expressed shock at the loss of so many lives in the blaze and asked authorities to conduct thorough search-and-rescue operations.

The Bangladesh Garment Manufacturers and Exporters Association said it would stand by the victims' families.

Bangladesh's garment factories make clothes for brands including Wal-Mart, JC Penney, H&M, Marks & Spencer, Carrefour and Tesco.

Separately, a flyover under construction fell onto a busy market, leaving at least 14 people dead including three construction workers in southeastern city of Chittagong, an official said Sunday.

Local fire official Abdul Mannan said the concrete structure collapsed on Saturday night, and authorities recovered the bodies by Sunday morning from under the debris in the second-largest city after Dhaka.

Laws don't curb pricey prostate cancer treatments

NEW YORK (Reuters Health) - Laws meant to prevent the overuse of expensive healthcare services don't stop doctors from using pricey prostate cancer treatments, according to two new studies.

Researchers found doctors used robots and special radiation to treat prostate cancer regardless of whether their area had laws requiring government approval before money is spent on healthcare facilities and new equipment.

"Certificate of need laws were designed to align public need with use of different services," said Dr. Bruce Jacobs, a lead author of one of the studies from the University of Michigan in Ann Arbor.

The U.S. government required states to implement the laws in the 1970s and early 1980s, but stopped a few decades ago. Still, some states continue to use the laws in an effort to control costs.

In each study, the researchers looked at treatments for prostate cancer, which is the most common cancer in American men.

The American Cancer Society estimates that one in every six men will be diagnosed with prostate cancer in their lifetime, but most will not die from it. Past research found that many men's prostate cancer is slow-growing, and most are candidates for active surveillance or "watchful waiting."

In Jacobs's study, the researchers looked at whether states with strict laws - those that require approval for even low-cost equipment - used robotic surgery to remove fewer prostates than states with less strict or no laws.

Jacobs and his colleagues write in The Journal of Urology that the price of such robots, and the questions surrounding whether or not robotic surgery to remove a prostate is better than the old-fashioned way should make it an "ideal target" for review under the laws.

In September, for example, one of the studies that have questioned the usefulness of robotic surgery found that men who had robotic surgery ended up having fewer short-term complications, but questioned its long-term benefits and whether the hefty price tag of $1.5 million in startup costs is worth it. But another recent study found robotic surgery led to fewer complications, fewer readmissions to the hospital, and fewer deaths due to surgery than traditional methods, according to Intuitive Surgical, the maker of the da Vinci Surgical System.

"That is significant for the patient and for reducing overall costs to the system," wrote Angela Wonson, a spokesperson for Intuitive Surgical, in an email to Reuters Health.

Overall, in the new study, the use of robotic surgery to remove prostates in Medicare patients increased regardless of whether there were strict, less strict or no laws in place. Also, the chance a surgeon used robots had nothing to do with the laws.

RADIATION AND COSTS

A second study by another group of researchers looked at whether the laws limited the use of intensity modulated radiation therapy (IMRT) or slowed the growth of healthcare costs related to prostate cancer.

IMRT allows doctors to focus radiation beams onto the cancer without harming healthy tissue.

However, the researchers write that IMRT is costly and - to their knowledge - has not been compared to other prostate cancer treatments in a randomized controlled trial, which is considered the "gold standard" of medical research.

In a group of Medicare patients, Dr. Ganesh Palapattu, the chief of urologic oncology at the University of Michigan and the study's senior researcher, found that areas with the laws actually saw greater growth in IMRT use.

Palapattu and his colleagues found that IMRT use increased from about 2 percent of all prostate cancer treatments in 2002 to almost half in 2009 in areas with the laws.

In areas without the laws, IMRT use increased from about 11 percent of all prostate cancer treatments to about 42 percent over the same time span.

The laws also didn't seem to help control prostate cancer treatment costs when the researchers compared the price to treat one person with prostate cancer in states with laws, compared to states without laws.

Palapattu told Reuters Health that it may be time to reevaluate the regulations.

"If the goal is to limit the overutilization of more expensive therapies and to improve efficacy or health, then we have to reexamine how we're doing this," he said.

Jacobs told Reuters Health that there is more research to be done, because his group's study did not look at how many applications for equipment may have been turned down by the states' approval board.

"I think if we really want to get to the bottom of how effective these (laws) are, the next step is to really look closely at each state's process of review," he said.

Palapattu said he'd also like to see if the findings are the same for non-Medicare patients. But, for now, he said men with prostate cancer should talk to their doctors about which treatment is right for them.

"Newer isn't always better, and it's important to have a meaningful conversation with your physician on treatment options and which one might be best for you and why," he said.
