Commissioners table golf cart vote ?indefinitely?

September 05, 2012 12:00 AM

The members of the Lenoir County Board of Commissioners agreed not to vote on a proposed countywide golf cart ordinance Tuesday.

Commissioner Eric Rouse, who brought the ordinance to his fellow commissioners during their Aug. 20 meeting, made a motion Tuesday to ?pull it at this time.?

The commissioners unanimously voted on a motion to ?table it indefinitely? ? County Attorney Bob Griffin asked to put the motion in more formal language.

The ordinance, which would allow golf carts to be driven on local roads by anyone age 16 and up who held a valid driver?s license, had prompted a great deal of discussion on Aug. 20.

The discussion included input from county Sheriff W.E. ?Billy? Smith and N.C. Highway Patrol Sgt. G.S. Hemby, who told the commissioners they would enforce any county ordinance, but cautioned their resources are stretched thin, and the golf cart ordinance could encourage riders to take major safety risks on the roads.

The carts would be allowed on roads where the speed limit is as high as 35 mph.

Other than the language of the motion, there was little to no discussion from commissioners Tuesday.

?Right now it was better just to hold off, was my take on it,? Rouse said Tuesday afternoon.

Rouse said he had heard comments for and against the ordinance in the community, but he was concerned about moving ahead without the full support of law enforcement.

He wanted more time for research and to give the public more time to comment before bringing it back for a vote.

?I just didn?t feel like today was the right day. . . . (I want to) just wait until I feel like it?s ready and I feel like the support?s there,? Rouse explained.

Joe South, Grammy-Winning Songwriter, Dead at 72

Joe South, the man behind many classic songs in the 1960s and 1970s, has passed away. He was 72.

According to The Associated Press, South's music publisher confirmed his client's death yesterday and cited an apparent heart attack at Joe's home in northeast Georgia as the cause.

Joe South

"He's one of the greatest songwriters of all time. His songs have touched so many lives. He's such a wonderful guy and loved by many," said Butch Lowery, president of the Lowery Group.

South performed on Aretha Franklin's "Chain of Fools" and Bob Dylan's "Blonde on Blonde" and wrote such hits as "Games People Play" and "Walk a Mile in My Shoes." He won the Grammys for Best Contemporary Song and Song of the Year for the former track.

Our thoughts go out to South's loved ones.

IADT: the still unanswered questions and choices that must be made ...

About 18 months ago, at the 2011 Genitourinary Cancer symposium in Orlando, Florida, Klotz et al. presented data from a large trial of intermittent (IADT) versus continuous androgen deprivation therapy (CADT) in the management of men with progressive prostate cancer after prior treatment with first- or second-line radiation therapy. We reported their data at the time.

The full report on the structure and results of this trial has now been published in? the New England Journal of Medicine. Unfortunately, only the abstract is available on line unless you are a subscriber to the journal. As far as we can tell, however, there is no significant difference between the published data and the data originally presented by Klotz in February 2011.

As many readers will already be aware, the results of this trial (that IADT was non-inferior to CADT in these patients) are in sharp contrast to the results of the trial reported by Hussain and her colleagues at the annual meeting of the American Society of Clinical Oncology earlier this year. It is not entirely clear at this time how to resolve these differences in outcome in the best interests of individual patients. It also needs to be realized, however, that there were some very real differences between the ways that these two trials were conducted and the eligibility criteria for participation in the two studies.

Clearly, there are men for whom IADT is not appropriate under any circumstances. Their disease is too aggressive and their PSA does not fall to a low enough nadir level during the first period on androgen deprivation, prior to any period off androgen deprivation.

The problem is whether there are men for whom IADT is entirely appropriate, how to pick these men out from the general population of men with a rising PSA who are candidates for androgen deprivation therapy, and how to then manage their intermittent protocol to optimize their outcome. There is a whole list of still unanswered questions:

  • Do these men need to exhibit a nadir PSA level of < 0.1 ng/ml after 6 or 9 months of ?lead in? androgen deprivation (or is any level < 4.0 ng/ml good enough)?
  • Do these men need to exhibit a nadir serum testosterone level of < 20 ng/dl after 6 to 9 months of ?lead in? androgen deprivation (or is any level < 50 ng/dl good enough)?
  • What is the most appropriate and effective type of IADT?
    • A brief period of an antiandrogen (e.g., bicalutimide) followed by an LHRH agonist alone during the ?on therapy? cycles but no treatment at all during the ?off therapy cycles? (IADT1)?
    • Continuous two-drug androgen deprivation with an LHRH agonist and an antiandrogen during the ?on therapy? cycles but no treatment at all during the ?off therapy cycles? (IADT2)?
    • Continuous three-drug androgen deprivation with an LHRH agonist, an antiandrogen, and a 5?-reductase inhibitor (e.g., dutasteride) during the ?on therapy? cycles but no treatment at all during the ?off therapy cycles? (IADT3)?
    • Continuous three-drug androgen deprivation with an LHRH agonist, an antiandrogen, and a 5?-reductase inhibitor (e.g., dutasteride) during the ?on therapy? cycles and continued treatment with the 5?-reductase inhibitor during the ?off therapy cycles? (IADT3+)?
    • Something else that we haven?t even tried yet (e.g., forms of IADT that include early use of drugs like enzalutamide)?
  • What is the most appropriate signal to restart androgen deprivation in a man with a rising PSA after a period off full androgen suppression?
  • What are acceptable nadir PSA and nadir serum testosterone levels after second, third, and subsequent ?on therapy? cycles of androgen deprivation?

We know that there are men who appear to do extraordinarily well on all the different forms of IADT. We also know that there are some men who started out with ?challenging? cases of progressive prostate cancer who appear to have done extraordinarily well on IADT3+ (and we fully expect at least one of those men to comment on this post). We also think that we know why the different forms of IADT fail, and it is the same reason regardless of type of IADT ? because the patients? cancers are progressing too rapidly into the realm of androgen resistance.

The problem is what we don?t know:

  • Why do the men who do well on the different types of IADT respond so well?
  • Are forms of IADT like IADT3+ truly superior to IADT1 and IADT2 ? or is this a mirage because we are only really aware of the most outstanding positive responses?

And then, of course, there is the issue of whether, if we take another decade or so to conduct studies that might be able to answer these questions, we will continue to get conflicting information.

So what does an individual patient do, faced with the current challenges? He is going to have to make a very personal choice after careful discussion with his doctors.

For men who want to be sure that they have the longest possible survival, regardless of side effects of therapy, this is a no-brainer: the Hussain-reported data and the data just published in the New England Journal of Medicine both suggest strongly that CADT with an LHRH agonist alone or with an LHRH agonist + an antiandrogen has an overall survival benefit when the responses of all patients who initially respond well to androgen deprivation are assessed (even if that that overall survival benefit is very small).

However, for those men who see quality of life as being a significant factor in their decision process, the decision is going to be much more difficult, and at this point in time we do not believe that there is clinically documented evidence that can categorically be used to demonstrate that any one form of IADT is superior to any other in terms of overall survival, prostate cancer-specific survival, or long-term quality of life. Furthermore, few clinicians actually have significant experience with the more sophisticated forms of IADT, which means that persuading one?s doctors to even consider such options can be ? in itself ? a challenge.

In the world of prostate cancer, the importance of becoming a well-informed patient who is able and willing to make tough choices is only becoming more evident. Your doctor may be able to guide you and help you understand and interpret the available data, but in the end only you may be able to make some of the actual decisions in the most appropriate manner that suits your personal goals.

Inside the Nokia Lumia 920's Camera: A Lesser Form of PureView, but Floating Lens Tech Means Smooth Video [Nokia]

Nokia's Lumia 920 flagship Windows Phone looks promising, but the camera isn't quite what everyone was hoping for. Where as Nokia's 808 PureView phone packed a massive 41-megapixel sensor and mostly served as a proof of concept for the PureView tech, the Lumia 920's sensor is 8.7-megapixels, which Nokia says can capture 5x-10x more light that other sensors. (AP) ? Rising country singer Hunter Hayes knows a thing or two about hurricanes.

The south Louisiana native has seen his share and even named his breakout single "Storm Warning" ?about a beautiful girl he compares to a "Cat 5" hurricane, the strongest category of tropical storm.

On Friday, the accordion-playing, harmonica-blowing singer from the Cajun town of Breaux Bridge returns, as Louisiana cleans up from Hurricane Isaac, for a concert benefiting a New Orleans hospital. He's also scheduled to sing the national anthem at the New Orleans Saints' football game against the Washington Redskins on Sunday, his 21st birthday.

"This is going to be like a huge homecoming celebration for me," said Hayes, who has just been named one of the CMA Award's new-artist nominees, along with Lee Brice, Brantley Gilbert, Love and Theft and Thompson Square.

The 46th annual CMA Awards will air live Nov. 1 on ABC from Nashville's Bridgestone Arena.

Hayes toured last year with Taylor Swift for part of her "Speak Now" tour and earlier this year with Rascal Flatts for its "Thaw Out" tour. Hayes hits the road again later this year as the opening act on Carrie Underwood's "Blown Away" tour.

"I'm living my dream," he said. "Music is my life. It's what I live for."

Though Hayes moved to Nashville about five years ago, he said he still considers Louisiana home. It's where he spent his childhood playing "every instrument I could get my hands on." That included the harmonica, accordion, drums and guitar, he said.

He frequently performed at Louisiana's Cajun, zydeco and seafood festivals but said he found his niche writing and performing country songs.

"Even as a little kid, I just related to country music," he said. "When listening to a country song, I felt like I was listening to somebody telling a story, and I really connected to that. I picked up on that and started writing, and it grew from there."

Hayes broke onto the country music scene last year with "Storm Warning." His other hits include "Wanted," ''Somebody's Heartbreak," ''Love Makes Me" and "Where We Left Off."

It's been a while since Hayes has performed in New Orleans. He said he hopes he can bring a little pick-me-up to an area still recovering from Isaac, which blew ashore Aug. 28, toppling trees and power lines across the state and leaving many without electricity. The storm also swamped much of south Louisiana with a tidal surge and dayslong heavy rains.

"That's one of the things New Orleans is famous for, a little music and a good time to get you through," Hayes said.

Hayes performs Friday night at the Mahalia Jackson Theater in New Orleans to benefit Ochsner Medical Center's pediatric unit. He said he has friends who work in the hospital's child care unit and "I love what they do." He's also looking forward to spending his 21st birthday at the Superdome for the Saints season opener, he said.

"I don't know how you can be from south Louisiana and not be a Saints fan," he said

Ex-police chief charged in China political scandal

BEIJING (AP) ? A former police chief whose flight to a U.S. consulate set off China's biggest political scandal in years has been charged with crimes including defection and bribe taking, possibly indicating the turbulent affair is moving closer to a resolution before a key national leadership transition this fall.

Wednesday evening's announcement by state media of the charges against Wang Lijun came hours before U.S. Secretary of State Hillary Rodham Clinton ended a brief visit to Beijing, and it was likely timed to convey to Washington that China's government is in full control and would strongly object to any foreign involvement in its internal politics.

Wang, the former police chief and vice mayor of the southwestern city of Chongqing, was also charged with "bending the law for selfish ends" and abuse of power, the official Xinhua News Agency said.

Wang set off the scandal by fleeing to the U.S. consulate in the nearby city of Chengdu in early February after being summarily demoted by the city's powerful Communist Party boss, Bo Xilai. Xinhua said the charges were brought by prosecutors in Chengdu, indicating that is where the trial will be held.

During his overnight stay at the U.S. consulate, Wang expressed to the Americans his concerns about the death of British businessman Neil Heywood in Chongqing last November. That prompted the British embassy to request a new investigation, which uncovered that he had been murdered. The case resulted in Bo's dismissal in March and the conviction last month of Bo's wife Gu Kailai for poisoning Heywood, a former family associate with whom Gu had reportedly feuded about money.

Gu was given a suspended death sentence while Bo remains under investigation by the ruling party's disciplinary branch for unspecified grave violations of discipline. Three leading Chongqing police officers and a Bo family aide were also sentenced as accomplices in the murder and subsequent cover-up.

Citing the indictment, Xinhua said Wang had left his post without authorization and defected to the consulate, but did not say whether he made a direct request for asylum, something U.S. diplomats say they would not have been able to grant.

Initially, critics of the Obama administration questioned whether it had let go a valuable defector. As a matter of policy, U.S. only grants asylum in American territory or at ports of entry. Embassies and consulates can only offer temporary refuge. U.S. diplomats have said that the policy was explained to Wang, who spent much of his 33 hours in the consulate on the phone negotiating with officials in Beijing.

Although he faces years in prison, Wang avoided the more serious charge of treason. Unconfirmed reports said he had cooperated closely with investigators after leaving the consulate accompanied by agents from China's main intelligence agency, the Ministry of State Security.

The indictment also said Wang knew that Gu was strongly suspected in Heywood's death, but "consciously neglected his duty and bent the law for personal gain" so she would not be held legally responsible. It said he abused his power in the exercise of police investigations and took advantage of his position to accept large amounts of bribes in return for granting favors.

The charismatic Bo had been one of China's most prominent politicians and was considered a candidate for the all-powerful Politburo Standing Committee at the upcoming Communist Party national congress, held every five years.

His March downfall injected new uncertainty into the succession process, although any damage seems now to have been contained. Vice President Xi Jinping remains on track to take over from President Hu Jintao at the congress, where seven of the nine members of the Standing Committee are due to step down after reaching retirement age.

Wang had been Bo's right-hand man in Chongqing, spearheading a controversial crackdown on organized crime that critics say featured torture and other violations of procedure, as well as illegal confiscation of assets and the targeting of political opponents.

The two reportedly fell out after Wang brought up Heywood's murder with Bo, who was not called as a witness or otherwise implicated in Gu's murder trial.

Xinhua's report also did not mention Bo, though Wang's indictment would seem to clear the way for an announcement about Bo's fate, something that had been expected well before the party meeting that is believed to be scheduled for mid-October.

However, University of Miami China expert June Teufel Dreyer said authorities might be holding back in the face of a lack of consensus or uncertainty how to proceed within such a tight time frame.

"The door's still open to prosecute Bo at a future time, and it is possible that Wang's trial will attempt to draw Bo into the murder-cover up intrigue," Dreyer said.

"The more attention can be diverted from so high-level, and reputedly fairly popular, personage, the better," she said.
